We work alongside the whole family to reduce barriers, strengthen understanding and create more joined up support across education, health, social care and community services.
Our support is available in many forms and is always person centred, helping families feel informed, supported and not navigating systems alone.
The project offers support across many areas including:
Direct support for children, young people and families, including guidance, advocacy and coordination with services to help navigate systems and access the right support.
A welcoming community space where families can access support, connect with others and take part in regular sessions including coffee mornings, parent support and local resources.
Support for professionals, organisations and local authorities through training, collaboration and commissioned work that strengthens inclusive practice and improves outcomes for young people.
Based within our Community Hub, the 5th Avenue Learning Hub provides EOTAS support for children and young people.
We deliver bespoke learning and development plans that combine life skills, wellbeing and academic learning, creating inclusive pathways where traditional education is not currently accessible.
Our approach supports children and young people to rebuild confidence, re engage with learning and access meaningful education in a way that works for them.
